Enrollment is OPEN to patients at the Cahaba Medical Care clinics in Centreville, Maplesville or Marion, AL who meet ONE of the following criteria:
– If you are African American (OR) Hispanic over age 40; (OR)
– If you are Non-hispanic White over age 50; (OR)
– If you are age 18 or older with diabetes (OR)
have had previously diagonsed glaucoma-associated disease (such as glaucoma suspect or ocular hypertension);
– If you are age 18 or older with a family history of glaucoma

Study Description
The University of Alabama at Birmingham focuses on the development of high-quality, accessible, and cost-effective strategies for eye care for at-risk patients. This research study seeks to implement and evaluate a telemedicine-based detection and management strategy for glaucoma and other eye diseases in patients seen at Federally Qualified Health Centers (FQHC’s) located in rural Alabama.


This proposal will evaluate portable measurement of visual function and optic nerve and retinal structure that are more applicable to rural locations with limited resources.
We will also identify and evaluate remediation strategies for the barriers to patient adherence with referral and follow-up appointments.
